- A signed copy of the Visa Declaration
- A signed copy of the Declaration Use of Mail Services
- An active passport with a minimum validity of 6 months
- Copies of the most recent biometric passport-sized photos (35x45 mm)

- Proof of postal address (Canada Post only)
